1. Mike Baxter of the Mets on August 4, 2012 at San Diego
2. Colby Rasmus of St. Louis on May 22, 2011 at Kansas City, but he got his fifth walk in the 10th inning
3. Milton Bradley of Texas on April 16, 2008 at Toronto, but he got his fifth walk in the 14th inning
4. Todd Helton of Colorado on April 29, 2007 home vs. Atlanta, but he got his fifth walk in the 11th inning
5. Edgar Martinez of Seattle on June 24, 2004 at Texas, but he got his fifth walk in the 18th inning
6. Jeff Bagwell of Houston on August 20, 1999 at Florida, but he got his sixth walk against the fifth different pitcher in the 15th inning
7. Mark McGwire of Oakland on April 26, 1997 home vs. Kansas City, but he got his fifth walk in the 11th inning

I'll do more later. Baxter is the only one of the seven players to do it in nine innings.

8. Cecil Fielder of Detroit on May 27, 1991 at Milwaukee, but he got his fifth walk in the 14th inning.

So far I've looked at the most recent 50 out of 100 times a player has had 5 or more walks in a game since 1918.

9. Elrod Hendricks of Chicago Cubs on September 16, 1972 home vs. New York Mets. Before Baxter, Hendricks was the most recent player to walk against five pitchers in the first nine innings of a game.
10. Hank Aaron of Atlanta on July 11, 1972 at St. Louis, but he got his fifth walk in the 15th inning
11. Gene Hermanski of Brooklyn Dodgers on September 22, 1949 at St. Louis. Hermanski did it in nine innings.

I went farther back to where play-by-play of games are not available and didn't find any more examples. So from 1945 on three players have walked against five pitchers in a nine inning game.
