Yeah, I currently have a herniated disk in my lower back as well and I can tell you that it is extremely painful when it flares up - which is seemingly sporadic. I can go for weeks and feel great and then suddenly it just locks up and I can barely walk. Now granted, unlike me, Phil has access to the Yankees medical staff so perhaps he can treat it with physical therapy (which I've been ignoring) but backs in general can be tricky.
David Ortiz tested positive for performance enhancing drugs in 2003.

. I have gone for therapy and found it to work. I also do a series of back exercise and found them to be helpful. Good luck!

Funny...off the top of my head, I would have thought that Clay Buchholz has had significantly more success in the majors than Phil Hughes. I compared their numbers though and it's incredibly close. Buchholz gets a slight nod due to era (Hughes 99 ERA+ and Bucholz 113+). Virtually identical winning percentage, games started, innings pitched. Hughes ahead on WHIP, but he pitched in relief for 40 games or so...

And yet Buchholz is probably considered the Sox best starter if not officially designated the ace...
